What is missing
What this does not tell you. It reports what is absent and what is dated oddly, because those are facts in your own file that anyone can check. It does not judge whether a note is any good, it does not score your workers, and it does not tell you whether you meet any standard. Nobody can tell you that from a spreadsheet, and a tool that claims to is handing you an opinion you would then have to defend.
What to do with this
Print this page and take it to whoever prepares your audit. The gaps above are the ones a sampler finds first, and every one of them is cheaper to fix now than to explain later.

Why these particular gaps
An approved quality auditor samples participant files. They pick a few, follow them through, and ask what happened on a given day. The failure the Commission writes up most often is not a badly written note, it is incomplete documentation: a shift with nothing against it, a note nobody's name is on, a record that cannot be placed in time.
Those are the things this looks for, and they are the things that are hardest to see from inside your own system, because an absence does not appear in a list.
